Con el termino nefrótico se entiende la incapacidad del filtrado glomerular de retener la proteína y el consiguiente pasaje en la orina (proteinuria). Cuando la perdida proteica en la orina es severa y prolongada, la proteína disminuye en la sangre, iniciando a recogerse líquido en los tejidos y hay un edema difuso. El paciente comienza a verse gordito, sobretodo en la cara, a nivel del abdomen y en los miembros inferiores. El peso aumenta de manera anómala y presentan escaso apetito. La gran mayoría de los niños nefroticos responden a la terapia con corticoide, aunque con el tiempo pueden presentar recidiva de la proteinuria, más o menos distanciada en el tiempo. Existen casos que no responden a los corticoides, que puede presentarse con el tiempo compromiso de la función renal y debe de realizarse la biopsia renal.
